Items are scored on a 7pt scale from 0-6

• 0 Not bothersome

• 3 “Somewhat bothersome”

• 6 “Extremely bothersome”

• Leg Pain

• Numbness or tingling in the foot or groin

• Weakness in the leg or foot

• Back or leg pain while sitting
