Greater Tubercle

  • Located on the lateral neck of humerus
  • Separated from Lesser tubercle via the intertubercular groove
  • The greater tubercle of the humerus serves as an attachment site for some of the tendons of the RTC
    • Supraspinatus
    • Infraspinatus
    • Teres Minor

Lesser Tubercle

  • Located on the lateral neck of humerus
  • Separated from Greater tubercle via the intertubercular groove
  • Lesser tubercle serves as an attachment site for part of the RTC (subscapularis).

Intertubercular groove

  • Provides a passage for the Biceps Brachii Long Head tendon to attach to the superior rim of the glenoid fossa

Anatomy

  • The angle of the walls of the intertubercular groove can vary widely
  • Separates the Greater Tubercle from the Lesser Tubercle
  • The roof is formed by the transverse ligament

Function

  • Allows the proximal tendon of the biceps Brachii Long Head to travel from its origin on the labrum and supraglenoid notch on the scapula, down the neck of the humerus and to the muscle belly

Dysfunction

  • Anatomic anomalies are associated with:
    • RTC tendinopathy
    • Bicipital Tendinopathy

Surgical Neck

  • The Surgical Neck of the humerus refers to the region inferior to the greater and lesser tuberosities, where the shaft of the humerus joins the upper margins.
  • The Axillary Nerve and Posterior Humeral Circumflex Artery run in close proximity to the medial aspect of the surgical neck.
    • Fractures at the Surgical Neck most commonly cause Axillary Nerve damage.
  • The Surgical Neck of the humerus fractures more commonly than the Anatomical Neck.

Anatomical Changes

  • Humeral Retroversion

Dysfunction

Fracture

  • Proximal humeral fx: Can have associated neural complications
    • Radial n: Wrist ext & Elbow str
    • Axillary n: Deltoid weakness
